The notification should include:
surname and first names,
date of birth,
voter place of residence (stay),
the number of a valid Polish passport or ID card, if you are in a European Union Member State, as well as the place and date of issue,
PESEL number, if the voter has one,
e-mail address, if the voter has one, to obtain confirmation of registration,
the voting circuit in which you intend to vote.
Applications may be submitted no later than 5 days before the day of elections to the Sejm of the Republic of Poland and the Senate of the Republic of Poland, ie on October 16, 2007 .
Voters permanently residing abroad who have been entered by the consul in the electoral roll may receive a certificate of the right to vote. With such a certificate, you can vote in any voting circuit in the country, abroad or on a Polish sea-going vessel. The application for a certificate of the right to vote shall be submitted to the consul who has drawn up the list of voters in which the voter is entered. The application should be submitted no later than on the 2nd day before the election day, i.e. by October 19, 2007.
A voter who has been issued a certificate of the right to vote will be deleted ex officio from the list of voters drawn up by the consul. Special care should be taken not to lose the voting certificate. In the event of its loss, regardless of the reason, it will not be possible to receive another certificate or vote in the circuit in which you were previously included in the list.
Voting to the Sejm of the Republic of Poland and the Senate of the Republic of Poland in voting precincts established abroad takes place at the premises of the district electoral commission during one day, without a break, between 6.00 am and 8.00 pm local time.
